This form is optional but helpful. Completing it before your first session can help your counsellor understand what matters to you. There are no right or wrong answers. You can leave any section blank, write “I’m not sure yet”, or only fill in what feels comfortable. If this feels overwhelming, skip it — we can explore goals gently in your first appointment.

1. What has brought you to counselling?

In your own words — only what feels okay to write. You do not need to share everything.

2. A preferred future (optional)

If things felt even a little more manageable or safer, what might you notice? Small, realistic changes count (e.g. sleeping a bit better, feeling less on edge with one person, leaving the house more often).

You’ve chosen to explore this in session — the writing box is hidden. Untick to write something now.

3. Broad goals (up to three)

What would you like to work on? Broad goals are fine. One is enough. You can also write “help me work this out”.

You’ve ticked “I’m not sure yet” — goal boxes are dimmed so there’s no pressure to invent goals. Untick to fill them in.

4. How might life look a little different?

For any goal above, describe a small real-world difference — what you might do, feel, or notice. Not about being “fixed”.

Difference boxes appear when you write a goal in section 3. Add a goal above to unlock the matching box here.

5. Pacing and safety (optional)

Anything your counsellor should know about pacing, topics to go slowly with, or what helps you feel safer in the room (or online)?

Thank you. We will treat what you share with care and confidentiality within the limits in your Confidentiality and Service Agreement. Goals can change — this is a starting point, not a contract. Return this before your first session if you can, or bring it with you.
